Registering For Online Services

Book appointments, order repeat prescriptions, view your allergies and adverse reactions and, if 16 years of age or over access your medical records.

Steps to registering for online access to your medical record:

  1. Download the patient information leaflet 
  2. Apply in person at the surgery with identification.

If you 16 years old or over you can request, in writing, to see, at the doctors discretion, more of your medical record on-line via Patient Access. Please allow 15 working days for this request to be actioned.

If a patient requires access to their detailed care record it is practice policy that they complete a registration form and return it in person with photographic ID (even if a patient already has online access to appointments and repeat prescription ordering).

If a patient requires a proxy to have access to their care record, A form at reception needs to be completed by the patient in the surgery and Photographic ID must be presented.
